New Student Requirements
Once admitted to one of the Occupational Therapy programs, you will need
to fulfill certain requirements before the semester begins. Without
these requirements completed you will not be allowed to commence
coursework. The requirements for the combined BS/MS and the Professional
Masters programs are listed below.
The asterisks denotes health documents that are in
addition to the health form you received when you were
admitted to Towson University. The Towson University documents
should be returned to the University Health Center. To access the health forms go to http://www.towson.edu/healthcenter/healthforms.asp
The Occupational Therapy documents should be uploaded to your personal Certified Background Profile. Please see instructions creating your Document Manager Profile in your acceptance packet to the program.

Towson University Health Documents
Please note that the Towson
University health documents you received after being accepted to
the university, should be returned to the University Health Center
by June 1, 2015
from any outstanding prerequisite coursework from Spring Term not taken at Towson University.

The following health documentation
a. current health insurance;
health insurance must be carried throughout the entire program (copy of insurance card front and back)
b. certification in infant ,child and adult CPR for Healthcare Providers with renewals required every 2 years (copy of CPR card front and back). Local CPR
c. Physical Examination form completed by healthcare provider.
d.. a completed immunization / health record/ with titers.
results of a 2 step P.P.D. test, Tspot, or Quaniteron Gold Blood Test
Titer results required for :Measles, Mumps, Rubella,
Varicella and Hep B.
** If titers results are negative updated vaccinations are required and then another titer completed. If Hepatitis B titer is negative you must complete at least the first two series of three Hepatitis B vaccines or completion of the Hepatitis B declination form **
IMPORTANT - These documents
must be uploaded to your Certified Background profile, by July 15, 2015.

Please note: If you are new to Towson University you will also need to submit the required university health documents to the University Health Center.
Fall schedule of classes
will begin to be emailed out the week of 7/27 to students who have turned in all of the above information and it has been processed. You will NOT receive your schedule until ALL documents are submitted and processed.
Due by End of
Completion of 3rd Hepatitis B Vaccine with titers results. Completion of a free hearing screening at the Speech-Language Hearing Center, Institute for Well-Being. You will recieve information regarding screening beginning of fall semester.
